Please place your order via email, fax, in-store, or by phone using the attached application form, which includes the following information: (organization (company) name, contact person's name, contact information, order details (name, quantity [Seafood Futomaki, Shrimp Lettuce Roll, Lucky Pork Cutlet Roll, Sweets Ehomaki]), pick-up date (February 2nd (Fri) or 3rd (Sat), whether you require delivery, preferred time (morning/afternoon), delivery location, and any other requests).
